This is where we roll up our sleeves and really look at what's going on — in your home, with your health, and in your daily life.
Dr. Lyndi comes to you for a 90-minute in-person visit. Together, you'll walk through your home and your routines, talk through your health situation, and identify what's working and what needs attention. Nothing is one-size-fits-all here. Every conversation, every recommendation, and every resource is tailored specifically to you and your situation.
You'll leave with a thorough understanding of your home safety, practical strategies for managing medications and daily function, and clarity on how to navigate the healthcare system — including what your doctors are recommending and why, how your insurance works, and what community resources are available to you. It's the kind of education and guidance most families never get, because no one has taken the time to sit down and explain it.
You'll also receive a written report with prioritized recommendations and a curated resource roadmap to guide your next steps.

Each month, you have a dedicated check-in call with Dr. Lyndi, direct email and phone access whenever questions come up, and up to four hours of care coordination — whether that means communicating with providers, tracking down resources, following up on referrals, or helping you prepare for an upcoming appointment. You don't have to figure it out alone, and you don't have to wait until there's a crisis to ask for help.

This is Dr. Lyndi's most comprehensive level of ongoing support, designed for aging adults and families navigating significant health challenges, major transitions, or layered care needs.
Each month includes a dedicated check-in call, direct email and phone access, and up to eight hours of hands-on care coordination. That might look like managing communication across multiple specialists, overseeing a transition home from a facility, coordinating with home care agencies, keeping family members informed, or advocating on your behalf when the healthcare system gets complicated. Whatever is happening, Dr. Lyndi is in your corner — proactively, not just reactively.
